Youth Retreat - April 12, 2025

Parent Consent Form

I request that my child be allowed to participate in the retreat Pilgrimage to Hope offered by the Diocese of Tucson.
Date: Sat, April 12, 8:30 AM registration; retreat time from 9.30 AM to 6.15 PM
Location: St. Cyril of Alexandria, [4725 E Pima St, Tucson, AZ 85712]
 

• My child is not younger than 14 years old at the time of this retreat.
• I will be responsible for the transportation.
• I have instructed my child to cooperate fully with the directions and instructions of the supervisory team in charge of the retreat.
• I have instructed my child to wear appropriate clothing.
• Should there be a medical emergency, 911 will be called. I agree that any cost or expense related to any emergency will be paid by me, by my insurance company, or any benefit plan of mine or that of my spouse.
• I understand in the case that my child is not feeling well, refuses to participate for any reason, or does not behave properly, I will be reached and will follow the directions given by the supervisory team in charge of the retreat. I will pick my child up before the end of the retreat if necessary.
• It is my responsibility to inform the organizers of any restrictions or accommodations my child may need, such as allergies or medical conditions. My child will be instructed to ask and avoid any food that may cause harm and turn over the organizers any special medication.
• My child will be expected to avoid the use of a cell phone, except for emergencies, or other electronic devices. I understand that my child is not required to bring any materials or items to the retreat. The diocese will not be responsible for any lost objects.

IMPORTANT:
I understand that photos will be taken during this retreat and that they may be used by the Diocese of Tucson to promote the event. (Teens will not be singled out in pictures without explicit consent. No information about individuals, such as names and ages, will be made public).
