Please remember that in order to administer benefits correctly the Diocese of Tucson Human Resources Department needs all new hires, status changes, compensation changes and terminations reported as soon as possible.

Any employee and or dependent losing the medical coverage benefit will receive the COBRA Information Packet directly from RETA Trust.

BENEFIT RATES FOR FISCAL YEAR 2008-09
The Benefit Rates Sheet (the rate sheet is the cost to the employer and to the employee) has the new rates the parish, school or agency will be billed for the fiscal year July 1, 2008, to June 30, 2009.

PREMIUM ONLY PLAN (IRS SECTION 125 -- PRE-TAX/AFTER-TAX)
The Premium Only Plan (Pre-tax/After-tax Authorization Form needs to be completed by all benefit eligible employees (Regular Fulltime 30 hours or more) for election per the IRS, Section 125. All employees who qualify for benefits should make an election of either pre-tax or after-tax regardless of what benefits are being elected. Any benefit eligible employee who does not return a completed form will elect that any premiums deducted during the fiscal year from their pay will be after tax by default.

BENEFITS ENROLLMENT/CHANGES
The Benefits Enrollment/Changes Form is for benefit eligible employees to make any changes, additions, deletions, cancellations and/or waive benefits during the open enrollment period. You may also use this form during the fiscal year for employees to elect benefits, make any changes, additions, deletions and/or cancellations during the fiscal year. Remember that employees can only make changes to the elected benefits during the open enrollment period, within 30 days of a qualifying event or when first becoming eligible for benefits. Any employee who is waiving any benefits as of July 1, 2008, must complete an enrollment form.
